The FUSE Committee brings together cross-system partners on a biweekly basis to coordinate housing stability strategies for a small group of individuals experiencing chronic homelessness and those with the most complex and acute needs. Through a by-name approach, partners align efforts, clarify responsibilities, identify barriers, and develop individualized action plans that leverage each organization's expertise.

Meetings are collaborative, action-oriented, and focused on practical problem-solving. Conversations center on what is currently in place for each individual, where coordination can be strengthened, and how partners can work together to improve housing stability and long-term outcomes.

We know that repeated involvement with emergency systems is both traumatic for individuals and costly for communities. Stable housing provides the foundation for improved outcomes, but lasting stability depends on coordinated partnerships and services matched to individual need. By working together across systems, FUSE helps ensure individuals receive the right supports at the right time, both during periods of crisis and as they build and maintain long-term stability.

FUSE welcomes professionals working to prevent and end homelessness, as well as those representing healthcare, behavioral health, corrections, public safety, and other community systems that regularly serve individuals experiencing housing instability. We also encourage participants to share this opportunity within their organizations and help identify additional partners whose expertise would strengthen this collaborative effort.
